I was wondering if someone could tell me what form component/module is being used on this site? and the easiest way to integrate it in with the extended menu...

The form component we are using here called "RSform". You can find it on extensions.joomla.org

Great! Yeah I saw it earlier, but tried that FacileForm (which looks powerful, but a little too much for what I need).
